Throughout my years of dealing with all sorts of situations, I've found it beneficial to maintain a Memo for Record on items that I think may benefit from such documentation.
Here, on hilsheimer.org, I've found an ideal location to maintain my MFRs. They can be updated at anytime from anywhere that I have an Internet connection... and they are a permanent record that can be referenced by anyone to better understand the history of those items I feel a need to document.
